This index includes parameters such as bowels and bladder control, grooming, toilet use, feeding, transfer, mobility, dressing, stairs, and bathing. The guidelines suggest recording what the patient does rather than what they could do, and scoring is based on the best available evidence. The minimum score is 0, and the maximum score is 20.

Guidelines to completing:
