I’m a speech therapist here to assess your swallowing and I’m part of the medical team here at _______. Before we begin, I’m just going to check your wrist band (confirm ID). Your doctor asked me to spend some time with you to learn more about your swallowing, eating and drinking. I’m here to make sure that it’s safe for you to eat and drink. I’ll be asking you some questions, looking at the muscles in your face and mouth, and observing you eat & drink a variety of things. This will take about 10- minutes. Is that OK with you?
