A root cause analysis will help you identify the source of any disruption or concern so you can take steps to avoid it from occurring again. To pinpoint root causes and arrive at a solution, the root cause analysis approach entails describing the problem, gathering evidence, and determining causal factors.
The 5 Whys approach is less formal than most more conventional problem-solving approaches, and it essentially entails wondering why over and over to get to the root of a problem or setback. Whether performed personally or as a group, this approach facilitates an open discussion that can spark new ideas about an issue. Each subsequent why expands on the previous one's answer. To keep track of the answers to the 5 Whys, start by filling out the example below; flowcharts and fishbone diagrams may also be used.
