Thought process Normal thought process. No flight of ideas. Thoughts are coherent and linear. Thought content He denies having any hallucinations, delusions, obsessions. No suicidal or homicidal ideations. Some intrusive thoughts R/T assignments.

  1. Cognitive Examination: Describe specific age-appropriate exam techniques that could be used to assess the cognitive status of your volunteer. Include the following components: Age of volunteer 19 Level of awareness or wakefulness The patient is alert and oriented to person, time, place, and situation. This was established using the MSE. Attention and concentration Good attention and concentration throughout the interview. The months backward test (MBT) may be used to assess cognitive impairment (Carlat, 2017). However, the pt was able to stay focused and answer questions appropriately during the interview. Additional testing was not required. Memory Good recall of events. Short term and long term

  1. Reflection: In 1-2 paragraphs, describe your therapeutic communication style in the recorded psychiatric history video in Week 4. What strengths did you identify? What areas require further development? I think my therapeutic communication style that I used in this interview was active listening. Active listening is about focusing on what the patient is saying - listening for the content, intent, and feeling of the patient to build trust in the patient and provider relationship (Tennat, et.al, 2023). I think I attempted to build trust by having good body language. I sat beside my patient which I think is less authoritative appearing. Additionally, I maintained good eye contact and smiled a lot. Another technique I used was summarizing. Summarizing demonstrates that I was listening to my patient and it also allows me the opportunity to document the conversation. After watching the interview, I realized that I never once asked my patient how he was feeling that day! I need to spend more time asking open ended questions to allow the interview to form more organically instead of asking all of my questions like I had an agenda. Obviously, some things got overlooked. I asked a lot of “yes/no” questions. By asking more open-ended questions I will signal genuine interest in my patients, making them feel valued and enabling me to better

understand them. Another opportunity for improvement is working on transitioning a little bit better. I think I tried using a smooth transition when my patient talked about caffeine use, but my response seemed a little forced and awkward. Reference: Tennant K, Long A, Toney-Butler TJ.
