A rare but life-threatening reaction to neurologic medications Characterized by fever , muscular rigidity , altered mental status and autonomic dysfunction

Defining NMS

NMS occurs only after exposure to a neuroleptic drug. Onset is 4-14 days after the start of therapy (10 days in 90% of cases). Once the syndrome starts it usually evolves over 24- hours.

 (^) Serotonin syndrome  (^) Malignant hyperthermia  (^) Malignant catatonia  (^) Meningitis/ Encephalitis  (^) Acute poisoning/ overdose (cocaine, Methamphetamine, Ecstasy)

DDx?

 (^) Emphasis is on supportive therapy (hydration)  (^) Discontinuing all antipsychotics is key  (^) Goal is to prevent complications (renal/ respiratory failure, rhabdomyolysis) Rx? It all goes aways in 2 weeks if you’re lucky enough though 
