Blocks reabsorption of of sodium and chloride in the
ascending loop of Henle. Excretion of sodium, chloride
and water are increased (increased urinary output)
losing potassium
Oral (tablet/solution), IM, and/or
IV. Give po with food. If BID,
give 2nd dose early afternoon.
Do not dilute IV meds but
administer slowly to prevent
ototoxicity. Protect from light.
Store oral solutions in
refrigerator.
Increased risk for digoxin toxicity with hypkalemia, increases effects
of other diuretics, NSADIS may decrese diuretic effect,
Neuromuscular blocoking agents are prolonged, lithum toxicity may
occur. Corticosteroids may increase risk for hypokalemia.
amini-glycosides may increase risk of ototoxicitiy
allergy to loop diuretics, hepatic coma, anuria,
dehydration, electrolye imbalance. Use caution in clients
with renal or hepatic disorders or DM. Older adults at high
risk for hypotension and electrolte disturbance
Electrolyte imbalances (sodium, chloride, potassium),
dehydration, hypotension, orthostatic hypotension,
ototoxicity may be permanent, hyperglycemia, increased
uric acid levels (susceptible to gout)
To treat pulmonary edema in heart failure,
edema caused by renal, hepatic, or cardiac
failure, hypertension, hyperkalemia
Assess serum electrolytes, assess for
signs of electrolyte imbalance, assess
BP and educate R/T orthostatic
hypotension. Assess for hearing loss,
vertigo, and tinnitus. Assess serum
glucose and uric acid levels. Consider
potassium replacement
Decreased pulmonary edema, decreased peripheral
edema, controlled hypertension
Report muscle twitching, confusion,
weakness, irregular HR, nausea.
Teach to monitor BP frequently and
avoid driving until stable. Report
hearing loss, ringing in ears, or
dizziness. Diabetics should monitor
glucose closely. Report symptoms of
gout.
Teach potassium dense foods
