Component of iCARE As an acute clinician nurse we perform dialysis treatments on patients in the hospital setting. We utilize primary team care model and it allows us to provide optimal direct highest care to our clients with great level of autonomy and quality. As part of a team care we collaborate together with primary nurse, nephrologists and other interdisciplinary teams to provide best quality of care to maximize optimal clinical outcomes. Moreover, providing care based on Compassion, Advocacy, Resilience, and founded on Evidence-Based Practice will highlight patient-centered care and improve collaboration with others. One of the improvements we can accomplish is our clinical setting is to be able to contribute in assisting patients in process of continuity of care and transition of the dialysis treatments from acute hospital to outpatient chronic setting.
. Compassion Compassionate care with collaboration is essential in nursing practice. It is not just related to patient’s clinical outcome and satisfaction, but also enhances alliance with other interprofessional multidisciplinary teams. compassion culture with other interdisciplinary team will increase positive engagement and fulfillment on both patients and healthcare providers. Advocacy Nurses hold critical role as patient advocates. Our job is unique due to spending most of our time to take care and interact with patients more than physicians. We have trust and therapeutic bonding with patients. As dialysis nurses our roles encompass education and advocacy providing patient with information and care guidance. We obligate to ensure patients voice and rights to be heard in regard their wishes or treatments. According to Levy (2018), in order for advocacy to be effective, nurses must maintain ethical awareness, self-confidence, persistence, pride in the profession, and maturity. We provide information and education on both treatments and help maintain patient values and autonomy; permit patients to make choice in what they believe to be their own best interests in regard of others opinions. On the other hand it will improve compliance and clinical outcomes. Another author suggested that advocacy in clinical setting will improve nurses knowledge and awareness on other people’s customs and tradition (Levy, 2018).

Resilience Nursing roles are multifaceted and being adaptive requires never ending learning and effective management of stress and adversity. Most important steps to develop flexibility and adaptability involves effective communication, stress coping skills, confidence in expanding knowledge base, ability to adjust easily, empowerment and conflict management. The nurses can be at the frontline leading interprofessional teams since they are direct care providers at the bedside and have most time with patients and their families. Nurses influence course of patients outcomes and play central role in transition and disease maintenance.
